Our population is aging. People are living with advanced chronic illnesses, complex needs and disability. It is increasingly difficult to know when they will die. How does your practice need to EVOLVE?

The Evolve Programme offers real solutions – through an interdisciplinary approach that integrates palliative care, rehabilitation and geriatrics.

Developed by Rebecca Tiberini, International Consultant and Coach and Chair of the EAPC Rehabilitation Taskforce, The Evolve Programme invites you to explore integrated solutions and, critically, to commit to implement these into your practice. This programme is facilitated in collaboration with St Christopher’s who will bring the concepts of The Evolve Programme to life through a range of successful interdisciplinary examples.
