What happens when a young person receiving paediatric palliative care becomes an adult? And how can both paediatric and adult palliative care services support a smooth transition and meet their changing needs? More young people with complex, life-limiting conditions are living into adulthood. Their transition from paediatric to adult services brings new challenges: for young adults and their families, as well as for the professionals caring for them.

This webinar brings together professionals from paediatric and adult palliative care to explore what young adults need around the time of transition, how paediatric teams can prepare them for what comes next, and how adult services can respond to this growing group.
